@Alex: UnitedHealth has a smoker surcharge. Fat surcharge would be good, but it's proving to be too hard/controversial to implement. BMI, for example, is not a good measure.

Kinda sorta…
BCBS has started a recent value add where somebody will call and offer to help out in tracking things, like appointments, procedures etc. (seems to be triggered by major procedures.) To help keep organized.

But overall? Outside of theoretically helping to cover the cost of major procedures, a little, but not much.
